Hi,
Questions to be asked in ESS from various heads could be segregated and presented separately. This segregation would help the employees to think in relevant terms and also enable employers to analyze the data, which could further be utilized to analyze exits and improve policies, compensation, benefits, trainings, etc.
Additionally, I would like to emphasize that it should be widely communicated so that the majority of employees are present. On the designated day, the entire organization should gather together, and within a specified timeframe, they should be provided with answer sheets. The completed sheets should then be collected promptly to avoid any discussions on the views expressed by individuals.
The analysis could also be shared with the employees to ensure the fairness of the system.

In these formats, you can ask the employees about their career progression, training & development sessions conducted by the organization, work culture, ethics, values, compensation, and benefit schemes.
