Need Help with Clearance Form Format for Employee Exit Process?

Hi all,

Could someone please send me the format of a Clearance Form? When relieving an employee from the company, we need to obtain clearance from various individuals and departments such as the CEO, Project Manager, Accounts Dept, Library, and Admin Dept (for submitting ID cards, cell phones, etc.).
