Quality Evaluation Form - Doc Download

I am posting this thing for the second time.
I am working in an international call center and I am looking for a quality evaluation form.
I want to include parameters like : convincing ability
Presenting skills
Opening call
Closing call
wrap up time
Voice modulations
Accent/Pronunciations
It would be good if i get to see some sample quality evaluation forms that are used in the call centers.

I am still waiting for your suggestions, can anybody please throw light on this matter?

i need a quality evaluation form..its for a call centre.i saw abhisheks but that is not something im looking for. something more simple that can grade intro, opening,pitch,closing and rebuttals. if someone has one ,can i have a look at it just to get an idea of how i can make one. thankyou.

It would immensely help to give a reply, if you could state whether you are a student or employee and what is the evaluation about and its purpose.
