Potential Apraisal Questionnaire

Hi all
I am doing a project on studying potential appraisal of employees in an organization.
Can you please provide me a sample of questionnaire on potential appraisal or can you please tell what kind of questions should I add in my questionnaire as soon as possible.

Could you share with me how is the procedure?. i noticed that the supervisor and reviewer has to sign in the form, is supervisor and reviewer is the same person?.
