Review After Probation Period

Hi
I am just setting down HR system in my company.
Can anybody solve queries on confirmation & appraisal.
1. Suppose a person is completing his probation period in the month of march'08. Then what are activity should be involved with HR dept.
Should Confirmation letter be given to him ? or Should Salary raise be consider as well ?
For these activity, do we need to fill any apparisal form ?
2. If persons is confirmed in the month of March'08. when his appraisal would be due. In next six months or one year?

As per our company policy, if someone is joining post March and our probation period is of 3 months wegive a raise after the review and if he is exceptionally good.
However if its a 6 month probation then year end is when it occurs.
Also the % of raise also depends.....
