I have sourced some good audit questionnaires from about.com.

HR Audit can be of 2 types.

Interal - within HR Dept.

External - how the Organization is serviced by HR Functions/services.

Start with placing a proposal plan as you will need Mgt buy in

1. Overview- Where your Co stands today, historical overview how it has aligned with its Vision/mission/strategies.

2. Objectives of proposed HR Audit (ot more than 5 max).

3. Strategy Implemetation road map. If you have Visio software, it will help to make graphic flow chart with time lie.

4. Report Submission: (Time line, deployment of resources)

5. Recommendations -supported by gap analytics.

6. Follow up - ensure that there is a evaluation of implementation of recommendations (yrly/biennal or quarterly based on solutions provided by you.

For questionnaire, I am developig one, will share once done. Check out about.com, they have very good ideas.
