Struggling with Duplicate Candidates in Recruitment? How Do You Track and Update Details?

I am working in an IT company as an HR executive in charge of recruitment. I need a format for updating the candidate details and checking for duplicates, as sometimes candidates are repeated without my knowledge. Kindly help me out with this.
