I am currently working on a project about Training and Development (T&D). Right now, I am in the process of preparing a questionnaire to identify training needs. I have already created one questionnaire but I am seeking additional examples that could assist me in developing a comprehensive questionnaire.

I have a questionnaire through which you can trace out the learning style of an employee. This will help you design the program for them. I will post it tomorrow.
