Even I am doing a project on T&D. Right now preparing questionnaire for identifying training needs. I have prepared one questionnaire but just looking for some questionnaires which will be of help to prepare a good questionnaire.

That was really helpful. However I am trying to prepare a questionnaire on identification of training needs.
I have already prepared one, just have a look at it. THere may be lot of mistakes which i need to correct. I have attached the one which I have prepared. So, Please let me know.
