Struggling to Understand Why Employees Leave? Need Help Finding Effective Survey Questions

I want to prepare a questionnaire for the employees to know the exact reason they want to leave the company. I saw some questions earlier on the site but now I'm not able to find them. Can someone please post them for me again?

Interview method is the best approach to understand the reasons why employees are leaving, instead of using a questionnaire.

As one of our friends said, an exit interview or outside counseling is the best way to find out the reason for leaving.

Employees leaving any organization will have issues specific to that organization. However, I am attaching a very broad-based questionnaire which you can use for your company after suitable modifications.
