Managing Employee Cab Services: How Do You Track Kilometers and Feedback Efficiently?

We provide cab facility for 200 employees. Can anyone suggest any format or any Excel sheet to maintain the records of kilometers, cab information, feedback, etc.?

Please see my feedback below:

Condition:
No. of shifts: 2
13 Cabs - 1st Shift
8 Cabs - 2nd Shift
No. of employees: 200

1) First name the cabs like S1R1, S1R2, S1R3... up to S1R13 by way of stickers pasted on the non-driver side of cabs should be clearly visible to all. Where, S1 refers to Shift 1, and R1 refers to Route No.1. Similarly, for Shift 2, also follow the same process like S2R1... up to S2R8.

2) You can have an Excel Sheet with the following parameters for tracking:
