Need Help Creating an Indent Form for Recruitment: How Do You Handle It?

Can someone help me with an indent form for recruitment? I will provide a situation to explain what exactly I require. Suppose the Mechanical Department requires an Engineer. Then they should give an indent for recruitment to HR, who will then proceed with sourcing the profiles and shortlisting them as per the requirements mentioned in the indent form submitted by the indenting department to HR. Can anyone help me with such a form?
