Creating a Dealer Agreement Form for UPS Business: Can Anyone Share Their Experience?

I am in the process of drawing up a "Dealer Agreement Form" for our company. We are involved in UPS business (UPS selling/servicing/installation). Can anybody help me with developing a DA form?
