Seeking Sample Performance Appraisal Form for Junior Management in Operations

Hi,

I am working for a ceramic tile manufacturing company. We would like to redesign a performance appraisal form for junior management employees working in operations. Please help me by sending a sample form from the building material industry.

Please talk to the functional heads and ask them how they would like to assess the performance of the operators. Align all the requirements into a presentation and present it to all the functional heads; fine-tune it accordingly. The idea is that the appraisal system should be adaptable to the unique needs of your company. A good Performance Management System (PMS) is one that is accepted by the line managers, and they should feel that HR is genuinely adding value to the PMS process.

Some important points to consider are:
At the operator level:
a) Attitude should be a component to be measured.
b) The measures should be quantifiable with both lead and lag indicators. This will help eliminate subjectivity in the appraisal process.
