Our hospital is a corporate one, and we are looking to update our Performance Appraisal format. Could anyone please share models of Performance Appraisal used in hospitals, hotels, or other service industries?

The formats can be downloaded from the net. You have to spell out what traits, skill sets, and competencies are important for your company. I feel the performance appraisal policy of each company has to do with its philosophy and needs and should not be replicated. Once you have identified the traits, skill sets, and competencies, it will become very easy for you to develop a comprehensive appraisal system.
