How Do You Gather Employee Details for Smooth Transitions After an Acquisition?

I am working as an HR for an MNC. Recently, we acquired another company, and their employees are transitioning into our company. We need to collect all the necessary details from them.

Please assist me with this issue. I require an Employee details format that includes personal information, addresses, and other details for PF, insurance coverage, and income tax processing.

10. Details of Family Members (Dependents)
10.1 Name
10.2 Date of Birth
10.3 Relationship
10.4 Occupation
11. Academic Records
12. Training Programmes Attended
13. Employment Details
14. Significant Achievements
15. Monthly Salary (CTC)
16. Joining Time
17. How he/she came to know about the job availability
18. References
