New to Performance Appraisals? Seeking Simple Grading Systems and Form Examples

Hi Everyone!

I've recently joined a company that is about to implement a performance appraisal system. Can someone please tell me more about the grading system and provide me with formats of appraisal forms for the same? I will not be able to implement the 180-degree or 360-degree appraisal system because it would be too complicated at this level.
