Requesting Salary Increment After Positive Performance Appraisal: Sample Letter Assistance

Hi all,

I have been working in a manufacturing company for the last 9 years as a sales and service coordinator. The company implemented the performance appraisal system through one software tool. I have received a 95% rating on my performance for the year 2007. I would like to ask the management for a salary increment. Can you please provide me with some sample letters?
