How are you? Thank you so much for your response. It's a general project that deals with both technical and non-technical people.

My theme is to explore the basic things that employees expect from training. I am interested in understanding how much the current training program satisfies their needs, such as general training programs like communication training and personality development. I am currently preparing a questionnaire for this purpose. I hope that my questionnaire will reveal the exact results, so I am seeking the help of you professionals. If you could provide me with some sample TNA questionnaires, I can further develop it. I am hopeful for a positive outcome from your assistance.

There is a lot of interesting stuff on www.businessballs.com. They also have a needs analysis form which is quite good. You can use that as a model and develop one, keeping in mind your needs. Do let me know if you need further help once you visit the site.
