Lost ESIC Card: Can We Reactivate an Employee's Old Number with a Duplicate Card?

Recently, one employee joined our company who has previously worked in a firm (that firm is now shut down) with ESIC facility. He mentioned that he lost his ESIC identity card but knows his number. The question is, can we activate the same number by applying for a duplicate ESIC card for the lost card? Kindly direct us on the procedure to be followed.

You can do the needful at your end. For this, you have to visit your ESIC local office where you can get all the details and procedures. You have to submit a new Form 3 in respect of the insurance number. The number will automatically be shifted to your code. Please keep all the correspondence regarding this for future reference because you may need these documents to show at the time of inspection or for any other purposes.
