A feedback form would be more appropriate for collection of Data, subsequent analysis and filing.
I am enclosing a simple feedback form that can be suitably modified to suit your purpose.

wat u can do is-> write topic name amd take rating on the topic on a scale of say, 1 to 5, where, 1=poor and 5=excellent and criterias can be interest generated, duration, clarity, coverage. and furher ask the participants to write their suggestions/comments.
