A feedback form would be more appropriate for the collection of data, subsequent analysis, and filing. I am enclosing a simple feedback form that can be suitably modified to suit your purpose.

What you can do is to write the topic name and take a rating on the topic on a scale of, say, 1 to 5, where 1 stands for poor and 5 for excellent. The criteria can include interest generated, duration, clarity, and coverage. Furthermore, ask the participants to write their suggestions/comments.
