How Do I Ask for a Salary Increase After Probation? Need Advice on Writing the Letter

Hi, I was on probation for a period of 6 months with the company I am currently working for. My probation ended in December. Could anyone help me with how to write an increment letter to my boss, and what percentage my salary should be increased by? Looking forward to your quick reply.

Clearly inform your boss about your time of joining, the designation you held, the awards or rewards you received from the boss, the important projects you completed, your educational background, any papers or external activities you participated in, and then mention that you are approaching for an increment as per the agreement.
