Seeking Interview Feedback Forms: How Do You Help Hiring Managers Evaluate Candidates?

I have joined a company as their Recruiting Manager. I am looking for various Interview Feedback Forms because I want to give my Hiring Managers the choice on the "type" of forms they would like to use. Presently, they do not have a systematic way to evaluate candidates.
