Yes, there is a format for payslip. I am attaching a typical payroll system that I developed. This is in accordance with Saudi system, you can change the formulas of this template according to your requirement and use it.

In case yours is under shop Act, then you can get the printed format of payslip from LAWPUBLICO,Abidds. otherwise computerised means, you can try for some specified models according to the software you are using.

As up to now there id n standard format for pay slip
the basic idea behind pay slip is to give full knowledge to employees regarding their working days, leave,salary components,deductions.and finally net payable;
if ur salary slip contains all of the above thats enough
