Does anybody have an Organization Health Check questionnaire to assess the present status? It's related to Organizational Development.

The Team Review Questionnaire, by Dave Francis and Don Young from their book "Improving Work Groups," is usable for both small and large groups.

The other alternative is working with a consultant company like Gallup, Trustmark, Hay, etc. If you need something customized, most good process facilitators should be able to customize a tool for your needs.
