SA8000 Audit Format: Does Anyone Have a Sample Questionnaire to Share?

Thank you, Kumanan, for your generous reply. Do you know about CSR? I am trying to establish the supplier CSR audit process. I came across a supplier of mine who mentioned that the SA8000 has some similarities in some manner with CSR. The elements that I cover during the audit are:

- Management
- Human Relations
- Environment Preservation
- Customer Relations
- Supplier Relations

What I intend to create are the questionnaires. I don't know if any of our friends here have a Corporate Social Responsibility Audit. Anyone?
