You are doing a project, which is good, but you want someone to help you out with the questionnaire. I would not appreciate that. You can have a sample questionnaire and work on it. I want you to think for your project and not depend on someone else. If you want, we all can guide you on how the questionnaire should be prepared, but again, you have to first research your GARMENT organization, its work policies, and the departments they have.

You need to first understand your organization and then think about the questionnaire. I have different types of questionnaires but I don't want to give them to you as you may become dependent on others for everything in the future. Think about it, and if you still can't find any solutions, then I will surely help you out.
