How Can I Resolve My EPS Claim Rejection Due to Missing Spouse Details?

I have retired from private service on 31-03-2024 at the age of 58 and claimed F&F from EPFO. Later last month, I claimed Form 10D for a monthly pension (deferred 1 year), but it got rejected with the following reason:

Claim Rejected: SPOUSE DETAILS NOT FURNISHED IN THE CLAIM FORM, SUBMIT ALONG WITH DOCUMENTS:: RETURNED FOR ATTACHING CANCELLED CHEQUE WITH MATCHING MEMBER'S NAME. VISIT FAQ ON PENSION ON EPFINDIA.GOV.IN.

How to Correct the Rejection

Kindly suggest how to correct the above rejection and the further action by considering my following points:

1. In 2019, I lost my wife due to health issues, so myself and my only daughter are my family members (no father or mother).
2. I nominated my daughter (19 years old) in EPS.
3. In Form 10D, I provided my daughter's details in the family section.

Also, guide me on whether any other declarations need to be uploaded to avoid rejection. I have a scheme certificate for 13 years (pensionable service 1995-2009) from a previous employer. I retired from my current employer at 58 after 14 years of service (2009-2024).

These are common errors that occur when you upload the requests. I am concerned that if the word "Spouse" is not present in the claim form, how can they reject a claim by stating that details of the spouse have not been provided?

Possible Reasons for Claim Rejection

The second reason may be genuine; you might have attached a cheque leaf with a different name or surname than what is available in the PF portal. It is advisable to visit the EPF Office and discuss the matter with the PRO, who is expected to assist the members.

Thanks, Madhu sir, for your immediate reply. As advised, I will attend PRO.

Regarding the name difference on the cheque, I was surprised because the same cheque was given for my EPF full and final settlement, and it was cleared without any rejection within 25 days.
