EPFO DSC Registration Repeatedly Rejected: How Can I Resolve This Document Issue?

DSC registration is being rejected multiple times even though all the documents were uploaded correctly. The required documents include Form 5A, a canceled cheque with the name, Aadhar Card, and Pan Card. Despite uploading these documents, the system continues to reject them, with the comments stating to upload the same documents again.

Kindly suggest how to proceed further with this issue. Thank you.

Please download a copy of Form 5A after updating and get it printed and manually signed. Along with the request for updating DSC (to be printed on company letterhead and signed), upload a copy of the Aadhar card of the registered signatory and the signed Form 5A. Ensure that the name of the signatory matches the Aadhar and PAN details. This will help you solve your query.

@techmin-consulting, thank you so much for your reply. I have uploaded the documents as you mentioned. However, I received a rejection stating, "REJECTED. REASON: proforma not uploaded." Can you please tell me what this means?
