EPFO Subcode Dilemma: Main Code Address or Branch Code Address for Form 5A?

Dear team, I need to create the EPFO subcode from the main code. I am trying to update Form 5A regarding this. I have a doubt about filling out the address of the branch. Should it be in the main code address or the branch code address? Please clarify this. Thank you in advance.
