Need Help with Maternity Leave Application and Doctor's Certificate Format?

I am Lakshmi, working in an insurance company. I want to apply for maternity leave, so please send the leave letter format and the doctor's certificate letter format as well.

Dear Lakshmi,

There is no fixed format available for applying for maternity leave. An application written on plain paper, along with the doctor's prescription mentioning the Expected Due Date (EDD), can be used to apply for maternity leave. However, please note that 6 weeks before and 6 weeks after delivery are applicable as per the Act.
