How Can I Effectively Request a Salary Increase After Leading a Major Project?

Dear all,

Could anybody please tell me or provide me with a good format of an increment letter? I am working with an insulation company in GCC as a planning engineer for the last 16 months. Although my designation is planning engineer, I have almost single-handedly executed a project worth almost 6 million USD. I strongly believe that compared to the standards of the insulation industry in other GCC countries, my salary is very low.
