I see this format is good, but what I suggest is to personalize the format. What you must remember in any resignation letter is that you should clearly write all your reasons as to why you are quitting the job. You can be very straightforward in your ideas if you are parting mutually, but then please don't forget to maintain a line between decency and straightforwardness.
