How Do I Write a Letter Confirming No Treatment Funds Were Given for an ESI Claim?

Hi all, One of my employees has completed treatment in a private hospital and is now approaching the ESI for the claim process. They are asking for a letter from us (the employer) stating that no funds have been given to the employee for treatment. Could you please assist me with how to write a formal letter or provide any format for this letter? Thank you.
