Break statement in service of the employee - he wants to get a scheme certificate

The employee left the job after 48 years. Now he wants to get a scheme certificate. He filled an online application for this, but the PF office rejected the application due to the non-submission of a break statement in service of the employee. Please guide me on how to prepare this statement.

Is there any long non-contribution period for that employee? If so, the employer needs to certify the NCP days, which I presume you mean by the term "break statement."
