One of our employees with basic salary is 30000 and he doesn't want to deduct his PF, is it possible?

Good evening, Please help to confirm, one of our employees with basic salary is 30000 and he doesn't want to deduct his PF, is it possible?
If yes in this case may I have to fill out any form?

Form II declaration form is mandatory irrespective of covered or un covered.

As you said as his EPF wages is Rs. 30000/- then check whether he was earlier covered under EPF. If so then you can't exclude if not covered then he may be excluded.
