Adding Father's Name to PF: What Should Be Included in the Employee's Affidavit?

Dear Sir, I need to add the father's name of one of our employees. I have submitted a joint declaration form along with Aadhar and PAN cards. Is there any declaration required from the employee's side on an affidavit? If yes, what information needs to be included in the affidavit for adding his father's name? Please advise.

A joint declaration itself is an affidavit, and nothing else is required to change the basic details.
