PF joint declaration form - what needs to be mentioned on the affidavit for adding his father's name?

Dear Sir,

I have to add father's name of once of our employee. I have submitted joint declaration form along with Aadhar and PAN card.

Is there any declaration from employee side on affidavit.

If yes, what needs to be mentioned on the affidavit for adding his father's name?

Please suggest.

A joint declaration itself is an affidavit and nothing else is required to change the basic details.
