PF & ESIC Account Creation - What are the mandatory documents required for registration?

Please help me to understand the process of Employer and Employee Registration for PF & ESIC. What are the mandatory documents required for registration? Help me to understand the process through a flow chart.

Also let us know the contribution part from both the employer and employee.

In respect of companies registered under the Companies Act, the ESI and EPF registration of establishment is automatic at the time of registration under the Companies Act itself. For others, you can do it using online portals. Once you have secured the employer RCs, you can register the individual members through the employer portal allotted to you.
