You can communicate very simply about the change of Mediclaim service provider. However in case of any change in Mediclaim Policy terms and conditions then the same needs to be communicated to employees. Just refer the below template
Dear Employees,
As you all know we have been providing complimentary Mediclaim insurance for all employees. Now due to some administrative reasons it has been decided to change the Mediclaim Service provider. Accordingly our new service provider will be TATA AIG with effect from_________. Our TPA will be_______. We will be issuing the new Mediclaim card shortly.
The terms and conditions for cashless and reimbursement option remain same. However please note that employees who are choosing reimbursement option should submit TATA AIG form which can be collected from HR Department.
Some of the pre existing exclusions are as below ( this you can add if applicable).
