ESIC Format for Changing Joining Date - what document are required?

As Per ESIC Rule Any Employee if joining in date of 17-04-2023 Then We have to Register his ESIC Within 10 Days. If we not register his ESIC within 10 day's of his joining date and what document are required to make the joining date at 17-4-2023 to the authority if any one have the format please shear the format

You have to reply to the show cause notice (as soon as you proceed with registration of an employee 10 days after his joining) with relevant appointment order and the joining report in support of joining. If you ignore it, the registration will be accepted but for all benefits the employee is entitled from the ESIC the date of registration will be taken.
