Dear Hari Patra, You have to submit a duly filled joint declaration form. You need to mention your incorrect Date of Joining (DOJ) and the correct DOJ on that joint declaration. You can process it through your current employer or previous employer.

When you submit FORM-13 (REVISED), your form may be rejected by the PF department. However, you can try 2-3 times, and your application will be approved, so there is nothing to worry about.
