I HAD WORKED FOR A COMPANY FROM JAN 1990 TO NOV 1995 AND LEFT THE COMPANY. AM I ELIGIBLE FOR GRATUITY. CAN I CLAIM NOW. IF YES PLEASE LET ME KNOW THE PROCEDURE.

Note that Payment of Gratuity Act-1972 protects an employee's right to a gratuity.
If employer refuse to pay gratuity amount then you can also file a complaint with the district office of the labour commissioner if your employer still doesn't provide you the entitled Gratuity.
