PF withdrawal - Employment period 2012-13 - Name mismatch

Requesting if you could support me with the process for withdrawal of PF.

I refer to the PF withdrawal for the employment period of April 2012 to March 2013. During that period, there happened to be no UAN. While I raise the withdrawal now, I am not able to get a proper response if I send the consolidated forms 19 and 10 C.

Secondly, my name is Praveen Gangadharan (The second name is my father's), but in PAN, it shows Gangadharan Praveen. My PF account shows as Praveen G.

How do I work on this to withdraw? It's almost a decade now. Kindly support me, please.
