How Can I Merge My Old EPFO Scheme Certificate with My Current UAN?

Dear Sir or Madam, I have obtained a Scheme Certificate from EPFO for my service period spanning from 15-07-1995 to 31-03-2006. Since 01-04-2006, I have been employed by a different organization and have registered a UAN with my current employer. I would like to integrate my Scheme Certificate with my existing UAN. Could you kindly guide me through the process?
