I would suggest to add in the appraisal format the following points :-
1. Number of weekly test conducted for the specific subject in a quarter.
2. Number of monthly test conducted for the specific subject half yearly.
3. Number of Mock test conducted for the specific subject yearly.
4. Analysis of performance by each student for specific subject in different classes and corrective action(s) for poor performers.
