School Teachers Evaluation & Teachers Appraisal Forms formats?

Please need help to make for my school Teachers Evaluation & Teachers Appraisal Forms. Can anyone please share any forms used in your departments? Thanks in Advance

I have attached the appraisal form for your reference, Hope it helps...

I would suggest to add in the appraisal format the following points :-

1. Number of weekly test conducted for the specific subject in a quarter.

2. Number of monthly test conducted for the specific subject half yearly.

3. Number of Mock test conducted for the specific subject yearly.

4. Analysis of performance by each student for specific subject in different classes and corrective action(s) for poor performers.
