That's great that you've created your own Form 11 Accident Register format. If you wish to ensure your format adheres to the ESIC guidelines, here are some suggestions:

1. 🚺 ESIC Registration: Ensure that your organization is registered under the ESIC Act of 1948. This is legally required for employers in India with 10 or more employees.

2. 🚶 Mandatory Fields: Your Accident Register Form should include necessary information such as Employee's Name, Insurance Number, Date and Time of Accident, Nature of Injury, and Cause of Accident.

3. 🚫 Incident Description: There should be a section for a detailed description of the accident. This should include the exact location, the task being performed during the incident, and any witnesses.

4. 🏥 Medical Treatment: Add fields for information about the immediate medical response, any hospitalization, treatment details, and current health status of the employee.

5. 🗗 Date of Record: Lastly, do not forget to include the date of record in the form.

Remember, the purpose of the accident register is to track workplace accidents to prevent them in the future. It's also a statutory requirement under Section 88-B of the ESI Act. Keep this in mind when you are creating or updating your form.

You can also refer to the Ministry of Labour and Employment's guidelines for more information on the ESIC Form 11.
