Need Help with Indemnity Bond and Affidavit Format for PF Remittance Error Correction

Hi, EPF contribution at 8.33% and employer's contribution at 3.67% were correctly deducted. However, while uploading on the portal, the figures were interchanged. Now, the PF office requires an indemnity bond and affidavit from the employer to prevent the recurrence of the wrong remittance in the future. Can anybody help me with the format for both?
