Opt-out option in PF - do we need to take any declaration from employees of nonpayment of PF (last 3 years)

Hi CiteHr Team, Our company started 4 years back, We are not registered PF to date,
We are planning to introduce PF next month,
do we need to take any declaration from employees of nonpayment of PF (last 3 years)
If yes pls provide the form

Were you liable to register and pay PF?
basically, did you have more than 19 employees at any time during that period?

Yes, we do have more than 20 employees

There is no provision for opting out.
You need to pay the PF in arrears, from the day on which you crossed 20 employees
