You may not find exactly what you are looking for, but there are plenty of examples for you to use and adapt to YOUR organisation's requirements.

It is a good question posed by you, and as suggested by our contributing colleague John there are a number of formats on this Forum. You can search and find out the one suitable for one.

2. It may be pointed out that requirements of qualifying criteria (SOW) is industry and work specific. Therefor you may need a customized QR/SOW for the company you are working, and for such customized QR/SOW you may send individual communication to me, if you like.
