How to withdraw EPS contribution (Pension amount)?

On the EPFO website, I am not able to see Form 10C or 10D.

I have been in four different organizations. All the pension contributions are still under each PF account. Only the provident fund contribution has been consolidated into my current organization's PF account. Is this how it works, or am I missing something? How can I consolidate all the previous organization's pension contributions into my current organization's PF account?
