From 10 in ESI approval - to date mentioned as "till date" by authority

Dear fraternity, The Form 10 in ESI for leave salary specified date mentioned on from date whereas to date mentioned as "till date" by authority.
How could an employer certify for leave salary such as till date and also employer shall approve any future date it could affect the employer at any cause?

Form 10 is to certify by the employer that the insured person has not worked during certain period due to medical reasons.

It cannot be mentioned till date, please verify it with the ESIC official.

It should be mentioned as exact match with the leave dates of the insured person in employer leave records.

Madam: but the ESI officer state that there is nothing to bother about approving till date which was mentined by authority.the leave may extent further so that mentioned as till day.what should do now.

Has the employee reported to duty? I hope you have submitted this form for processing his sickness leave wages.

If this is the case it cannot be mentioned as till date as the employee already reported to duty.

From date and To date is must. If the employee has reported in the middle days in the be mentioned in the space provided.

Madam: first thank you for your guidance which is much helpful to sort the issue .however,we approved so because employee and his family are in economical crisis .let discussed about it and came to this decision moreover no liability affect anymore as which is directed by ESI office.
